Event Liability Waiver

Event: Community After Hours
Location: CSH Fitness, 15504 Stoneybrook W Pkwy #110, Winter Garden, FL 34787
In consideration of being allowed to participate in the Community After Hours at CSH Fitness, and in recognition of the inherent risks associated with the event, including the serving and consumption of alcohol, I hereby agree to the following terms and conditions:
Acknowledgment of Risks: I acknowledge that participating in the event and consuming alcohol carries inherent risks, including but not limited to accidents, injuries, or illness. I understand these risks and assume all responsibility for any personal injury, property damage, or other loss that I may sustain as a result of my participation in the event.
Alcohol Consumption: I understand that I will be provided with a maximum of two (2) drink tickets for alcohol consumption during the event. I agree to consume alcohol responsibly and acknowledge that no alcohol will be sold at the event. I understand that CSH Fitness and its partners reserve the right to refuse service to any attendee who appears to be intoxicated or poses a risk to themselves or others.
Release of Liability: I hereby release CSH Fitness, its owners, affiliates, employees, and partnering businesses (hereafter referred to as "Released Parties") from any and all liability, claims, demands, damages, actions, and causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by me or any property belonging to me, while participating in the event or while on premises where the event is taking place.
Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all losses, liabilities, damages, expenses, and costs (including attorneys' fees) resulting from my participation in the event, my consumption of alcohol, or my actions or omissions during the event.
Acknowledgment of Policy: I confirm that I am of legal drinking age and agree to comply with all local laws and regulations regarding alcohol consumption. I understand that my participation in the event is contingent upon my adherence to this policy and the responsible consumption of alcohol.
Photography and Media: I acknowledge that photographs and/or videos may be taken during the event. I hereby grant CSH Fitness and its partners the right to use and publish such media for advertising, promotional, or any other lawful purposes.
Understanding and Agreement: By signing below, I confirm that I have read and fully understand this Liability Waiver. I acknowledge that I am signing it freely and voluntarily, intending to be legally bound by its terms.
